GW SON Survey Policy
Policy Statement
Investigators who wish to survey SON faculty, SON staff or SON students must have the approval of the GW Nursing Office of Research prior to distributing the survey.
Reason for Policy
This policy will enable tracking of survey requests and ensure that when a survey is distributed it has been vetted for scientific merit, protection of human subjects, and has a response burden that is concomitant with its value.
Who is Governed by this Policy
All those who endeavor to survey GW faculty, staff or students, including surveyors internal to GWSON or external to the School. This includes requests from students from other Universities. The only exception is for standard/regularly occurring surveys, such as course, exit or follow-up graduate, and university mounted by the University and distributed centrally.
Policy

All requests to survey GW SON faculty, staff, or students require vetting by the Associate Dean for Scholarship, Innovation and Clinical Science.

The surveyor should respond to the questions attached to this document, in writing, and include a copy of the proposed survey. This should be emailed to the Associate Dean at least 10 business days prior to the proposed survey date.

No survey will be mounted without written permission of the Associate Dean. 4. Once the survey is complete, a report of the results will be provided to the Associate Dean within 1 month of distributing the survey.
Definitions
Survey: any questionnaire (paper or electronic) or interview schedule where SON faculty, staff or students are the intended sample.
